We strive to provide appropriate treatment for each patient based on current standards of care and procedures updated through regular continuing education. We offer confidential treatment rooms with up to date equipment in a modern, comfortable office which is maintained to the highest infection control standards. To ensure proper diagnosis and appropriate treatment, we rely on periodic radiographs, visual inspection of the teeth and soft tissues, and other diagnostic tests as needed. Our office operates on an appointment only basis. We make every effort to honor our patient's appointment time and stay on schedule. When provided with proper forms, we will file charges incurred with your insurance carrier. Please be advised that our relationship is with you, not your insurance carrier. Fees and knowledge of coverage are ultimately the patient's responsibility.
To minimize waiting time, please complete the patient information sheet and medical history form provided. Bring this form, along with a current insurance card to your initial appointment. Please arrive 15 minutes prior to your scheduled time to allow sufficient time for our staff to establish your computerized account. A Parent or guardian needs to accompany minors to the initial visit. Please make our staff aware of any medical conditions which may affect your treatment. Because failed and/or broken appointments can be costly and disruptive, we request the courtesy of a 24 hour or more notice if you are unable to keep your scheduled appointment. Failure to provide sufficient notice may necessitate a missed appointment fee or the inability to schedule further appointments.
