What to expect as a new patient
We strive to provide appropriate treatment for each patient based on current standards of care and procedures updated through regular continuing education. We offer confidential treatment rooms with up to date equipment in a modern, comfortable office which is maintained to the highest infection control standards. To ensure proper diagnosis and appropriate treatment, we rely on periodic radiographs, visual inspection of the teeth and soft tissues, and other diagnostic tests as needed. Our office operates on an appointment only basis. We make every effort to honor our patient's appointment time and stay on schedule. When provided with proper forms, we will file charges incurred with your insurance carrier. Please be advised that our relationship is with you, not your insurance carrier. Fees and knowledge of coverage are ultimately the patient's responsibility. We are currently preferred providers for Delta Dental, Parkview Health Plan Services, Guardian, Aetna and Cigna networks.
What you can do to help
To minimize waiting time, please download the patient information sheet and medical history form by clicking the button below. Complete it before your appointment and bring this form, along with a current insurance card, to your initial appointment. Please arrive 15 minutes prior to your scheduled time to allow sufficient time for our staff to establish your computerized account. A parent or guardian needs to accompany minors to the initial visit. Please make our staff aware of any medical conditions which may affect your treatment. Because failed and/or broken appointments can be costly and disruptive, we request the courtesy of a 24 hour or more notice if you are unable to keep your scheduled appointment. Failure to provide sufficient notice may necessitate a missed appointment fee or the inability to schedule further appointments.
Insurance and Financial Matters
Our relationship is with you, our patient, and not your insurance carrier. Our fees are determined reasonable for this geographical area and are covered by most insurance plans to the percentage level they apply. Charges for services rendered are ultimately the patient's responsibility regardless of insurance coverage. We ask that you pay the patient portion at the time services are rendered. This may include co-pays, deductibles, or the full fee when there is no contractual agreement with an insurance carrier. Payment options include cash, check, or money orders. We also accept Discover, VISA, and MasterCard for your convenience.
We realize that occasionally financial concerns do arise. We encourage patients to contact our billing staff for assistance in the management of your account. Regardless of divorce settlements or custody decrees, we consider the adult patient being treated or the parent bringing a minor child to be responsible for fees incurred. Submission of a pre-determination of benefits for complex treatment is generally recommended. As a service to our patients, we will gladly file insurance forms, however it is impossible for us to know your plan's potential coverage.
